Once you’ve decided to be our guest at Fuze Nights, we need you to complete the Guest Registration form. Do note that this might take you about 15 minutes to complete but this will help us collect all the info we need to make your appearance a great one!

Points to Note

• You will need your bio - you can either write one, link to one online or upload one.

• You will need a decent looking photo we can use in promo materials.

• You will need to sign a Guest Release for the event. You can review one here first: SAMPLE RELEASE.

Fair Commitment

We ask that you commit to the dates as firmly as possible. We respect everyone has busy schedules and our team is no different. We do appreciate that circumstances beyond your control happen, so please make sure you contact us as soon as you know you won’t be able to participate at our event.
